Through Medicap Pharmacy, Carbondale Area School District is able to host a COVID vaccine clinic for students who are currently ages 16, 17, and 18 years old. Recently, a new waiver has allowed the administration of the COVID-19 Vaccine to current 16, 17, and 18 year old students. The vaccine that will be administered is the Pfizer vaccine.
The clinic will be held from 11:00 AM to 2:30 PM THIS Friday, April 30, 2021, for students in Cohort B, and from 11:00 AM to 2:30 PM on Monday, May 3, 2021, for students in Cohort A. Students who are receiving instruction 100% virtually, may attend the clinic either day.
Permission forms MUST be received by Thursday, April 29, 2021, at 11:00 AM, and should be returned to Mrs. Priscilla Bilski, high school nurse. Additionally, parents are welcome to attend the administration of the vaccine and should note the request on the school district’s waiver form.
